With a new administration firmly in place and efforts to repeal and replace the Affordable Care Act (ACA) underway, health systems find themselves confronting newfound uncertainty. In light of a market that is shifting more proactive patient management outside the hospital and demanding higher-value care- not just high quality care- the conventional quality path that most hospitals are on is no longer sufficient.
Health System leaders increasingly look to the medical neighborhood to link specialists with primary care medical homes. Providing patients with the comprehensive, seamless experience expected of a medical neighborhood requires continuous communication, including collaboration on care plans and clearly defined patient management responsibilities. Highlighting case studies from provider networks across the country, this presentation will give an overview of the medical neighborhood model and tactics for enhancing coordination within the neighborhood to improve patient access and reliability of care.

- Review the importance of coordination, enhanced access, and continuous accountability within the medical neighborhood
- Analyze how providers advance care team capabilities and processes to facilitate PCP-specialist collaboration and in-network referrals
